APPLICATION FOR RENEWAL OF ESTABLISHMENT LICENSE

Application for the renewal of Establishment License will be fully online by next year via our new upgraded online system MeDC@St 2.0. However, because of some technical difficulties we have to postpone the implementation until further notice.

Meanwhile, to facilitate the establishment to renew their establishment license, we provide establishment license renewal application process manually.

Please download the Application Form and complete the Form and submit to MDA office together with all the required documents. Please refer to the guideline for more information.
